Oral nicotine pouches and lozenges are a new category of tobacco product that includes brands such as Zyn, On! and Velo and they come in many flavors. The use of flavoring in tobacco products has been proven to appeal to youth, and sales data show that nicotine pouches are increasing in popularity. According to Nielsen data reported in an industry news article, Zyn nicotine pouch dollar sales in convenience stores increased 470% in the first half of 2020. Truth Initiative has also observed a concerning number of young people reporting use of oral nicotine products. For example, Truth Initiative tracking shows that 13% of 15-24 year olds surveyed in Fall 2020 were past 30-day users of pouches. Enrollees in This is Quitting, a text message quit vaping program for young people from Truth Initiative, are also reporting use of nicotine pouches when they sign up for the quit program.


While official data on youth use do not yet exist because they are not included in national surveys, Truth Initiative has submitted recommendations that federal agencies include oral nicotine pouches and lozenges like Zyn, On! and Velo in future surveys. As more information emerges, here are some important things to know about these products.


Oral nicotine products are, as yet, not regulated as tightly as smokeless tobacco products or combustible tobacco in the U.S. The FDA places some regulations on these products, including requirements that manufacturers submit certain information to the agency, use nicotine warning labels and comply with some basic marketing restrictions. No current regulations exist that prevent or restrict flavored nicotine pouches and the marketing restrictions on these products are not as strict as those on smokeless or combustible tobacco products.


While specific health effects of nicotine pouches like Zyn remain unknown, youth use of nicotine in any form is unsafe. While these products are not technically categorized as smokeless tobacco, they have many similarities to smokeless products, which carry serious health risks.


Chronic use of smokeless tobacco can result in nicotine addiction and there is evidence that smokeless tobacco use leads to cigarette use. According to nationally representative data published in National Center of Biotechnology Information, non-tobacco using youth who try smokeless tobacco are more willing to try cigarettes and e-cigarettes one year later.


Truth Initiative strongly urges the FDA to remove all unauthorized oral nicotine products from the market, including Zyn, that the agency has not authorized for sale. In order to prevent youth from using these products, eliminating all flavors, including menthol, from all tobacco products is crucial, as well as instituting marketing restrictions on these products to prevent youth exposure to them.


The most common location for tobacco pouch keratosis is in the lower anterior vestibule followed by posterior vestibule. The lesion appears thickened and corrugated, and as the condition worsens, it becomes more leathery.[3] Literature reveals that this condition has been established as a risk factor for developing oral carcinoma. However, they are said to have a lower risk of malignant transformation when compared to leukoplakia. Chronic use of smokeless tobacco can result in nicotinic dependence. Further, researchers have proved that use of tobacco during adolescence can disrupt brain circuits and affects cognitive abilities, attention deficits, and psychiatric disorders.[4]


Many carcinogens have been found in smokeless tobacco and the major causative agent being tobacco-specific nitrosamines, and other agents include nitrosamino acids, polycyclic aromatic hydrocarbons, aldehydes, and different types of metals.[5] In addition to the risk of oral cancer, smokeless tobacco has been associated with the development of gingivitis, gingival recession and attachment loss, halitosis, reduction of taste, and abrasion of teeth.[6] However, there was no evidence of taste disturbances in our patient.


The Tobacco and Cancer Program of the National Cancer Institute recommends that all health-care professionals follow the 5As guidelines to aid in successful intervention of educating and counselling tobacco users [Table 2].[7]


There is a range of different tobacco smoke-free pouches out there including dip, chew, and snuff. Dip pouches and chewing tobacco mean that there is a build-up of juices that you will need to spit out at some point. However, there are also nicotine pouches that mean you don't have to spit at all! The long-term health impact of nicotine pouches is still unknown. Not technically categorized as smokeless tobacco, these products are not yet regulated as strictly by the Food and Drug Administration (FDA) as smokeless tobacco products or combustible tobacco are in the United States. Long-term data is yet to come, so we are unsure how reduced exposure may translate into reducing harm to our health. No current regulations exist that prevent or restrict flavored nicotine pouches and marketing restrictions are not as rigid as those on smokeless or combustible tobacco products.


There is no data to show nicotine pouches as a safe or effective way to quit. "The FDA does not approve these products as tobacco cessation interventions or nicotine replacement medications, so I would not recommend them," says Jill Selzle, PA, Nebraska Medicine certified tobacco treatment specialist. "If someone is already using nicotine pouches and has been able to quit tobacco, I would suggest weaning off them with the goal of nicotine freedom entirely."


"My best advice is to pay attention to what you are saying to yourself," says Selzle. "We believe what we tell ourselves, so make sure you are saying the right things. You can do it and you will do it! What I want for my patients is for them to be their best self and I can confidently say that tobacco is not a part of that plan."

We present the clinical findings and treatment of haemangiomas and arteriovenous malformations only, although many other vascular lesions affect the lip. We used simple clinical findings for our diagnosis and treated our patients by applying the Tabaksbeutelnaht (tobacco-pouch suture) technique to encircle the tumour and strangle it. Dissolvable forms of smokeless tobacco come in different shapes and sizes, such as tobacco lozenges, orbs, pellets, thin strips (like melt-away breath strips), and toothpick-sized sticks.
